Clinical decisions about medicine use should be a shared decision between patient and healthcare provider and should involve consideration of risks and benefits. Such conversations are not possible where the evidence informing risks and benefits is not available. This is frequently the case for complex and understudied populations such as pregnant and breastfeeding women and their infants. This lecture will provide an ethical framework and case for inclusion of these groups in clinical trials together with some examples of studies and community engagement work.
